when you apply for a green card(adjustment status)in the united states , you are required to undergo an immigration medical examination. the medical examination required by cdc regulation is a means of evaluating the health status of aliens applying for entry to the united states . the examination must be done a doctor who is designated by US citizenship and immigration services(USCIS). USCIS designates certain doctors(known as civil surgeon)to perform the immigration medical examination required for most green card applicants .

DURING THE EXAM
The immigration medical exam entails a review of your medical history and a physical examination .the civils surgeon will complete a comprehensive examination that includes review of medical history and physical examination. he will also test for communicable diseases like tuberculosis, syphilis, and gonorrhea as required by the the center of disease control and prevention (CDC).
the civil surgeon will review your vaccination history and go over the vaccination requirement
WHAT TO BRING TO THE EXAMINATION
1- I-693 FORM (please fill out parts 1-4 and bring to your exam appointment to save time . or we will give you the form in the office and will help you to fill it out
2- government issued photo identification such as valid passport, driver license. possible forms of identification include your birth certificate(with english translation)
3-vaccination or immunization record (if you are missing some of the vaccination , we provide you to some of the immunization on site
4-form of payment (cash or credit cards) . we do not accept insurance or check
LABORATORY :
YOU WILL BE SENT TO TO GET YOUR TEST TO QUEST, LABCORP OR BIO REFERENCE DEPENDING ON YOUR INSURANCE OR FORM OF PAYMENT . RESULTS MAY TAKE 3-5 DAYS
